Please use the post box at the car park entrance at Chiswick Health Centre to drop in repeat prescription requests. Please write your nominated pharmacy details on the script request. This post box is secure and cannot be accessed without a key.

Please register for the online services to order your medication without coming in to the surgery. You can do this by calling in to the prescriptions line and verifying your information.
